Please type out the Entry Form online before printing and snail mailing the 16 page document to the address indicated on the form. If you have any questions regarding registration, please email tomjay@chukajudo.org with the subject: registration. Questions regarding Shiai and Kata should be directed to 2008CJI@chukajudo.org with the appropriate subject.

ALL ENTRIES ARE NON-REFUNDABLE

Please allow extra time for U.S. Postal Service mail delivery – Tournament Entry forms will be processed in the date order in which they are RECEIVED, not postmarked.

We recommend clubs send their players tournament application packets batched together to ensure the same receipt date for all of their players.

Mail-in Pre-Registration RECEIVED on or before Thursday, May 8, 2008

Shiai: $50.00 Second division $30.00
Kata: $40.00 per team. Both Katas $60.00 per team

Mail-in Registration RECEIVED on Friday, May 9 and no later than Thursday, May 22, 2008 Absolutely NO entries will be opened if received after the Thursday, May 22, 2008 cutoff date. Entry will be considered late and will be returned to sender.

Shiai: $75.00 Second Division $55.00
Kata: $60.00 per team. Both Katas $80.00 per team

The non-refundable entry fee must accompany the official entry form with the required documents. Personal checks, cashier’s checks or money orders are acceptable. A $30.00 fee will be charged for all returned checks to be paid the day of registration, plus all late fees charges, CASH ONLY.

Walk up registration for Saturday, May 31 tournament, open from 5:00 pm until 7:00 pm on Friday, May 30, 2008 at the Tournament Headquarters.No entry will be accepted after 7:00 pm Friday, May 30, 2008.

Walk up registration for Sunday, June 1 tournament, open from 12:30pm until 2:30pm on Saturday, May 31, 2008 at the Tournament Headquarters. No entry will be accepted after 2:30 pm on Saturday, May 31, 2008.

Pre-Registered Contestants Check-In:

Check-In: Upon arrival at the tournament headquarters, pre-registered contestants can report directly to the Lower Atrium for check-in. Competitors must present their actual USJF, USJA (show proof of insurance), or USJI card. Those without a valid card will be required to purchase one at that time. Once checked-in, report to the weigh-in areas:

• Males Weigh-In - Report to the Whitney Room
